Linked-Care Program Information

Linked-Care is a collaboration between Dignity Health and El Hogar Community Services. Through the El Hogar Linked-Care program, you will be linked with mental health providers to services bridging the gap between the client and provider connection. We provide behavioral health counseling to individuals of all ages experiencing mental health challenges.

How Linked Care Can Help:

• We will call you to schedule your appointment and give you a reminder call. Transportation services are available if needed.
• Our clinician will discuss your needs, concerns, and challenges at your first appointment. They will work with you to determine and link to appropriate community service(s) to receive regular and ongoing support that addresses your needs.
• Support will be provided until you are fully connected with your new provider.
